Job summary

Complete Care Management is hiring a part-time LSW Licensed Social Worker Assistant to support residents in a long-term care setting. The role emphasizes psychosocial assessments, care planning, and emotional support for individuals transitioning from hospital to long-term care.

Responsibilities include advocacy, grievance resolution, discharge planning, and connecting residents with community resources. Must be licensed LSW and able to pass background checks.

Qualifications

• Prior Long Term Care experience is required.
• Licensed to work within the State of Connecticut as a LSW Licensed Social Worker.

Responsibilities

• Conduct psychosocial assessments for new residents to evaluate emotional and social history.
• Develop and maintain individualized care plans with the healthcare team.
• Provide emotional support to residents dealing with grief, loss, depression, anxiety.
• Advocate for resident rights and ensure needs are heard by the care team.
• Assist with grievance resolution by mediating complaints and reporting abuse or neglect.
• Coordinate discharge planning and connect residents with home health, hospice, or community resources.
• Assist residents and families with legal, financial, and insurance case management.
• Guide residents and families through advance directives.

Here’s What You’ll Do for Our Residents & Facility:

• Conducts Psychosocial Assessments: Evaluates a new resident's emotional, social, and psychological history.




• Develops Care Plans: Collaborates with the healthcare team to create and maintain individualized care plans & updates the interdisciplinary team on resident progress and goals

• Emotional Support: Provides counseling to help residents cope with grief, loss, depression, anxiety, and the transition to a nursing home.




• Advocacy: Protects and promotes resident rights, ensuring voices are heard and needs are met.




• Grievance Resolution: Mediates complaints and reports any suspected abuse or neglect.

• Discharge Planning: Coordinates safe discharge plans, connecting residents with community resources like home health care or hospice.




• Resource Connection: Assists with connecting residents and families to legal, financial, and insurance case management.




• End-of-Life Support: Guides residents and families through advanced directives
