Job summary

Chelsey Park Health & Rehab in Georgia is seeking an LPN to provide high-quality nursing care in a compassionate, mission-driven setting. You will monitor skin conditions, administer medications, document patient progress, and communicate with the supervising RN.

The role includes discharge planning, admissions, and coordinating care with physicians, with opportunities for wage supplements and benefits. Candidates must hold a Georgia LPN license and a nursing degree or equivalent and demonstrate

Qualifications

• Nursing degree or LPN program completed.
• Active LPN license to practice in Georgia.
• Strong organizational and communication skills.
• Ability to read/write and follow procedures.

Responsibilities

• Monitor skin problems and treat as ordered by physician.
• Administer and record all treatment medications.
• Chart patient progress, ailments, meds, symptoms and observations.
• Coordinate with supervising RN on patient progress and care.
• Provide reports to Director of Nursing for updates and actions.
• Identify, manage and treat wounds and skin lesions.
• Maintain communication with attending physicians.
• Admit, discharge and transfer patients as requested.
• Send patients to hospital when necessary.
• Perform compliance rounds and report issues.
