Job summary

Qualifications

• Graduate of accredited school of nursing.
• Current licensure with the State Board of Practical Nurse Examiners, with no restrictions.
• Membership in professional and/or community organizations desirable.
• CPR/BLS required
• TB and Flu vaccination

Responsibilities

• Performs nursing procedures and direct patient care under supervision of a MD/NP/PA.
• Observes, records and reports changes in patient’s condition.
• Administers prescribed medications and treatments per plan.
• Assists the MD/NP/PA in performing specialized procedures.
• Prepares equipment and materials adhering to aseptic/sterile technique.
• Assists patient with daily living activities and wound care.
• May assist with Home to Heal home visits.
• Implements nursing components of the care plan.
• Documents progress notes in EMR; follows MD/NP/PA orders within scope of practice.
• Completes paperwork and mandatory documentation daily.
• Travel may be required to various clinic locations and home visits.
• Performs wound care and dressing changes as directed.
• Maintains dress code and professional appearance.
