Job summary
Edgefield Post Acute is seeking a licensed practical nurse (LPN) to provide high-quality nursing care to residents in a skilled nursing setting. The role includes administering medications, monitoring patient conditions, documenting care, and collaborating with physicians, therapists, CNAs, and the interdisciplinary team to achieve optimal outcomes.

Qualifications
- Must possess a current, active LPN license in the applicable state.
- Experience in skilled nursing or long-term care is a plus.
- Strong assessment, communication, and teamwork abilities.
Responsibilities
- Deliver high-quality nursing care to residents with compassion and professionalism.
- Administer medications and treatments as ordered by physicians.
- Monitor residents and report changes promptly to RN/physician.
- Document care accurately in the medical record.
- Collaborate with physicians, therapists, CNAs, and the care team.
- Supervise and support CNAs and maintain resident-centered service.
