Job summary

Chartiers Center, Inc. is a Pittsburgh-based non-profit organization seeking an experienced LPN to deliver compassionate health support within a community-based behavioral health setting.

You will provide integrated care, wellness coaching, and care coordination across homes and community locations, coordinating transportation and documenting care. PA LPN licensure with 3 years of behavioral health experience is required.

Qualifications

• Licensed Practical Nurse (LPN) with PA licensure and 3 years behavioral health experience.
• Valid driver's license required.

Responsibilities

• Build positive, supportive relationships with individuals and assist them in achieving their health and wellness goals.
• Utilize evidence-based wellness coaching to empower individuals to participate in managing their health and care.
• Deliver services in homes, community locations, healthcare facilities, and other provider sites.
• Coordinate and, when necessary, facilitate transportation to medical appointments using agency vehicles.
• Review and analyze behavioral and physical health data to identify needs and develop wellness plans.
• Monitor progress and provide follow-up support to promote adherence to treatment and care plans.
