Job summary

Person-Centered-Supports, LLC is seeking a Licensed Practical Nurse for a full-time, first-shift role near Pocono Lake, PA. The LPN will support a multidisciplinary team, monitor member health, and deliver wellness education in a compassionate community setting.

Shifts are Monday to Friday, 12:00 PM to 8:30 PM. The team emphasizes quality care, collaboration, and adherence to mission and core values while documenting care and coordinating with the medical team.

Qualifications

• PA license from an accredited LPN program.
• Current LPN license in the state where the job is performed.
• Complete listing of all health care settings where you worked as an LPN.
• 3 years of experience with psychiatric and forensic populations; CPR certification.
• Valid, non-commercial driver's license with clean driving record.

Responsibilities

• Provide health and wellness services for all members.
• Assist psychiatric team with medication issues and monitoring.
• Provide screenings and referrals to PCP.
• Assist in coordination-of-care with the psychiatric team.
• Lead health education, groups, and wellness programming.
• Document day-of-service activities.
• Collaborate with pharmacy for medication accuracy.
• Attend daily Morning Team Meetings and weekly Team Meeting.
• Perform weekly medication room maintenance.
• Complete monthly medication room checklist.
• Record monthly vitals and weights for members.
• Participate in inter-agency meetings as needed.
• Complete regulatory trainings as required.
