Job summary

Artis Senior Living of Wilmette is seeking an LPN/LVN to join our care team as a Coordinator of Health & Wellness. The role emphasizes clinical monitoring, care planning, and supporting residents with dementia in a compassionate setting.

Responsibilities include medication management, daily care, assessments, and supervision of Caregivers and CNAs. Our team values safety, dignity, and exceptional resident experience in a Great Place to Work Certified environment.

Qualifications

• Must possess current LPN/LVN designation as granted by State Board of Nursing.
• Experience in rehabilitative or geriatric nursing is preferred.
• Fulfill continuing education credits as required by state.

Responsibilities

• Take an active role in resident assessments, development of service plans, and daily care.
• Oversee and implement the self-administered medication management program for residents.
• Administer medication and/or treatment to residents in accordance with physician orders.
• Document and report care provided and resident observations including changes in condition.
• Provide emotional support, physical comfort and ensure safety of residents.
• Supervise Caregivers and CNAs in providing resident care per guidelines.
• Assist in marketing the community by tours and relations with referral sources.
• Assist with resident move-ins and acclimation to new surroundings.
• Perform other duties as required.
