Job summary

Trilogy Health Services, LLC is seeking an LPN to provide direct clinical care and supervise nursing assistants on the unit. The role includes daily resident assessments, reporting changes to the care team, and ensuring medication and supply readiness.

You will perform administrative tasks, monitor safety, and respond to emergencies as needed. Weekend shifts are included with the Weekend Package incentive and additional evening/night differentials.

Qualifications

• Must have and maintain a current, valid state LPN license.
• Current, valid CPR certification required.

Responsibilities

• Visit residents on assigned unit daily to observe and evaluate each resident’s physical and emotional status and notify charge nurse, physician, or DON of changes.
• Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
• Provide holistic care for residents within the scope of your LPN license.
• Assist in monitoring inventories of medications, supplies and equipment to ensure adequate stock.
• Perform administrative duties such as completing medical forms, reports, evaluations, and charting.
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals per policy.
