Job summary

Fairmount Homes, Inc. in Ephrata, PA is seeking a Licensed Practical Nurse (LPN) to join our Skilled Nursing team, providing high-quality, resident-centered care on a beautiful rural campus.

This role encompasses administering medications, monitoring health, and coordinating with the interdisciplinary team to ensure safety and comfort for older adults. The LPN will supervise nursing assistants, assist with admissions and end-of-life care, and support infection prevention and regulatory

Qualifications

• PA LPN license in good standing
• Current CPR certification required
• Strong clinical judgment and decision-making skills
• Ability to work independently while collaborating with the care team
• Genuine compassion and a desire to serve older adults

Responsibilities

• Administer medications and treatments per physician orders and nursing standards
• Monitor resident health and respond to changes in condition
• Make rounds to promote safety, comfort and quality of care
• Document care accurately and communicate updates to the interdisciplinary team
• Supervise and support nursing assistants while promoting teamwork
• Assist with admissions, transfers, discharges, assessments and end-of-life care
• Support infection prevention, resident safety and regulatory compliance
• Build meaningful relationships with residents and families through compassionate care
• Participate in team meetings and quality improvement initiatives
