Job summary

Valor Healthcare, Inc. is seeking a passionate Licensed Practical Nurse (LPN) to join the Community Based Outpatient Clinic team in Kosciusko. The position offers a weekday schedule with competitive pay, excellent benefits, and generous time off.

This PRN role collaborates with the VA and PACT teams to deliver high-quality care to veterans. The LPN will perform prescribed nursing tasks, assist with care plans, and ensure accurate documentation in the patient chart.

Qualifications

• Diploma or certificate of an accredited practical/vocational nursing program.
• Minimum one-year experience as an LPN/LVN in a primary care or ambulatory setting; government healthcare environment preferred.
• Must be credentialed and in good standing with VA.
• Must have a valid unrestricted license in the state of employment or as required by VA contracts.
• Current Basic Life Support (BLS) certification; ACLS as required by contract.
• Proficiency in written and spoken English.
• Strong computer skills; EMR experience.

Responsibilities

• Assist with patient care, physical exams, and routine tests.
• Perform nursing assessments and routine preventive health services per VA and clinic protocol.
• Administer medications as directed and document doses.
• Operate diagnostic equipment and maintain patient records.
• Assist with front desk duties as needed and collaborate with VA and non-VA providers.
• Provide patient and family education on self-management, prevention, and healthy lifestyles.
• Maintain patient confidentiality and comply with training requirements.
