The role includes overseeing admissions, discharges, and transfers, assessing residents, administering medications, wound care, and coordinating care plans with the interdisciplinary team.

Responsibilities

• Directs the daily activities of the licensed practical/vocational nurse and CNAs in accordance with current regulations and facility policies.
• Participates in admission, discharge and transfer of residents; assesses changes in status and communicates with physicians and families.
• Oversees medication administration, wound care, and other direct care tasks per orders.
• Schedules follow-up appointments and coordinates transportation as needed.
• Provides nursing leadership to on-unit staff and collaborates with the interdisciplinary team.
• Participates in management team meetings and survey processes; supports infection control and QAPI activities.
