Job summary

Life Care Centers of America is seeking an LPN Unit Care Coordinator to supervise and coordinate patient care on the unit, ensuring compliance with laws and Life Care standards. The role requires an active LPN/LVN license and involves leadership of nurses and CNAs on the floor.

The ideal candidate has completed an accredited LPN program, CPR certification, and prefers at least one year of geriatric nursing experience. Must maintain licensure and uphold professional conduct.

Qualifications

• Attended an accredited LPN program.
• Active LPN/LVN license in the applicable state.
• One year geriatric nursing experience preferred.
• CPR certification required on hire and kept current.

Responsibilities

• Direct the daily functions of unit nurses and CNAs on the floor.
• Chart appropriately, accurately and in a timely manner.
• Provide, manage and coordinate patient care to support well-being.
• Accurately prepare and administer medication as ordered by a physician.
• Lead in emergency situations related to patient and staff safety.
• Coordinate care plans with the supervising RN.
• Demonstrate excellent customer service and a positive attitude.
• Assist in evacuation of patients and maintain attendance.
• Communicate effectively on an interdisciplinary team.
