Job summary

Life Care Centers of America is seeking an LPN Unit Care Coordinator to supervise and coordinate patient care on the unit, in coordination with nursing staff and families. The role emphasizes leadership, accurate charting, and safe medication administration to ensure high-quality care.

Required are an accredited LPN program and an active LPN/LVN license; CPR certification is required or obtained on orientation.

Qualifications

• Attended an accredited LPN program.
• Currently licensed/registered in applicable State. Must maintain an active LPN/LVN license in good standing throughout employment.
• CPR certification upon hire or obtain during orientation. CPR certification must remain current during employment.
• One (1) year geriatric nursing experience preferred.

Responsibilities

• Direct the daily functions of unit nurses and CNAs to provide leadership on the floor.
• Chart appropriately, accurately, and in a timely manner.
• Provide, manage, and coordinate patient care and services through interpersonal contact to attain or maintain patients' well-being.
• Accurately prepare and administer medication as ordered by a physician.
• Respond in a leadership capacity to emergency situations related to patient and staff safety.
• Coordinate patient care plans and services with RN.
• Exhibit excellent customer service and a positive attitude towards patients.
• Assist in the evacuation of patients.
• Demonstrate dependable, regular attendance.
• Communicate and function on an interdisciplinary team.
• Read, write, speak, and understand the English language.
