Job summary
Mental Health Association in Massachusetts is seeking an LPN to support the GRIT co-occurring programs within the Recovery Services division. The role blends clinical nursing with recovery-oriented, person-centered support in community residential settings, including medication oversight and care coordination.
The LPN will collaborate with the Nurse Manager, prescriber, and a multidisciplinary team, advocating for participants’ health and timely follow-up care while contributing to discharge
Qualifications
- LPN license in Massachusetts is required.
- At least one year of applicable experience preferred.
- Ability to coordinate care and advocate within the healthcare system.
Responsibilities
- Assess physical health needs and shape individualized care plans.
- Monitor medications, educate participants, and observe side effects.
- Coordinate care with external providers and various teams.
- Provide staff training on medication-related tasks and procedures.
- Support treatment planning, discharge planning, and wellness initiatives.
- Lead teaching sessions with participants, one-on-one and in groups.
