Creekside Center for Rehabilitation and Healing in Tennessee is seeking an LPN to provide post-acute care across physical, occupational, and speech therapy programs. Qualifications

• Must possess a nursing degree from an accredited college or university, or be a graduate of an approved LPN or RN program.

Responsabilités

• Medication administration and treatments.
• Assist with admission, transfer and discharge of residents
• Document resident care services in medical records and department records
• Educate residents and family members regarding treatment and rehabilitation
• Adhere to infection control policies and facility policies
• Consult with resident's physician regarding plan of care and changes
• Maintain resident confidentiality and privacy
• Ensure quality of care by following DOH and facility standards
• Collaborate with interdisciplinary team to resolve resident issues

Formation

Nursing degree or diploma from an accredited college or university; graduate of an approved LPN or RN program
