The GEO Group seeks a Licensed Practical Nurse - Telehealth Coordinator to support facility patient care via telehealth, coordinating appointments with community providers and ensuring proper documentation in the electronic health record.

The role requires an LPN license, two years of experience, and familiarity with telemedicine equipment. You will prepare the clinical environment, assist with patient forms, and contribute to data reporting while working onsite.

Qualifications

• Graduate of an accredited nursing program with LPN licensure.
• Two years of LPN experience preferred.
• Familiarity with telemedicine equipment and processes.

Responsibilities

• Coordinate all medical telehealth appointments with the designated scheduler.
• Prepare the clinical environment and equipment for remote care.
• Assist patients with standardized forms and history for telehealth.
• Liaise with community providers to share vital signs and info.
• Provide requested medical data from EMR to providers during encounters.
• Assist with documentation and records in the EMR system.
• Prepare monthly telehealth statistics and support routine duties.
