Qualifikationen

• Current license and registration to practice as a Licensed Practical Nurse in New York State.
• Ability to provide nursing care within the LPN scope of practice under RN direction.
• Ability to direct, support, and work with CNAs and care team.
• Strong verbal and written communication skills.
• Solid problem-solving and decision-making abilities.
• Accurate documentation and use of clinical computer systems.
• Caring, patient, and respectful approach to residents.

Aufgaben

• Administer medications and ordered treatments safely and accurately.
• Manage the daily clinical care of assigned residents and short‑stay patients.
• Monitor residents and promptly report symptoms or changes to RN or provider.
• Follow residents' plans of care and contribute necessary clinical information.
• Direct and monitor CNAs on the assigned team.
• Provide coaching and hands‑on assistance to CNAs as needed.
• Maintain nursing notes, medication administration records, and other docs.
• Safely store and account for medications and controlled substances.
• Communicate professionally with residents, families, providers, and the team.
• Support infection prevention, safety, regulatory compliance, and respectful care.
