Job summary

Elderwood at Cheektowaga is actively seeking Licensed Practical Nurses to join our skilled nursing and rehabilitation team in Cheektowaga, NY. This role offers full-time, part-time, and per diem opportunities with competitive hourly pay.

LPNs will provide direct patient care, coordinate with CNAs, and contribute to a high-quality clinical environment. You will manage daily care for residents, administer treatments, and collaborate with RNs and physicians to ensure optimal outcomes.

Qualifications

• Current license in New York State to practice as an LPN.
• Ability to provide nursing care within LPN scope and under RN direction.
• Experience directing, supporting, and working with CNAs.

Responsibilities

• Administer medications and ordered treatments safely and accurately.
• Manage daily clinical care of residents and short-stay patients.
• Monitor residents and communicate changes to RN or provider.
• Follow resident plans of care and maintain accurate notes and records.
• Direct and monitor CNA duties and provide hands-on guidance when needed.
• Maintain documentation for medications and treatments and support infection control.
• Communicate professionally with residents, families, and interdisciplinary team.
