Job summary

StoryPoint is seeking a Licensed Practical Nurse (LPN) for a part-time role on the 3rd shift (10P-6:30A) at their Crystal Lake location. This position requires a qualified LPN with experience in Assisted Living or Memory Care, as well as prior supervisory experience. Qualifications

• Licensed Practical Nurse (LPN) in the state.
• Assisted Living/Memory Care experience preferred.
• Prior supervisory experience required.

Responsibilities

• Lead, manage, and train your team of caregivers.
• Responsible for evaluating residents and reporting changes in condition to physicians.
• Utilizing Electronic Health Record (EHR) and Electronic Medication Administration Record (EMAR).
• Communicating effectively to team, residents, and families.
• Building strong relationships.
