Job summary

Liberty Care and Rehabilitation Center is seeking a dedicated LPN/LVN to provide direct nursing care to residents, including medication administration and wound care. You will supervise nursing assistants, monitor resident status, and help coordinate admissions and discharges in a patient-centered environment.

Requirements include an active LPN/LVN license in Kentucky, 1+ year of clinical nursing experience (long-term care preferred), and strong documentation skills.

Qualifications

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum 1 year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong clinical observation, medication administration, and documentation skills.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs, ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during shift per policies.
• Monitor residents' status and notify physician or responsible party of changes.
• Develop and review individualized resident care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
