Liberty Care and Rehabilitation Center in Kentucky is seeking a dedicated LPN/LVN to join our compassionate team. You will provide direct nursing care, perform medication administration, and support residents with daily living activities in a culture that prioritizes dignity and quality of life.

The role includes supervising nursing assistants, maintaining accurate documentation in our EHR, and collaborating with an interdisciplinary team to develop personalized care plans.

Qualifications

• Current, active LPN/LVN license in Kentucky.
• Minimum one year of clinical nursing experience; long-term care preferred.
• Supervisory or charge nurse experience preferred.

Responsabilités

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs and ensure comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during the shift in accordance with policy.
• Monitor resident status and notify physician of changes.
• Develop, implement, and review individualized resident care plans with the interdisciplinary team.
• Document care accurately in the EHR per policy.
• Coordinate admissions, transfers, and discharges as per procedures.
• Participate in quality assurance activities and staff education.
• Perform other duties as assigned by the Director of Nursing.
