Job summary

Signature HealthCARE is seeking a compassionate LPN/LVN in Monroe County Rehab & Wellness Center to provide direct nursing care, administer medications, and support residents' daily needs in a long-term care setting.

The role includes supervising nursing assistants, documenting care in the EHR, coordinating admissions and discharges, and participating in quality assurance. Active LPN/LVN license in Kentucky required; CPR/BLS preferred.

Qualifications

• Active LPN/LVN license in the state of practice (Kentucky).
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with electronic health records (EHR) systems and accurate documentation.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) ensuring comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during the shift.
• Monitor residents’ status and notify physician of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Document care accurately per facility policy and procedure.
• Coordinate admissions, transfers, and discharges.
• Participate in QA activities and staff education.
