Job summary

Applied Home Health Solutions LLC is seeking an LPN Program Manager to provide clinical oversight and daily operations for residential group homes. You will lead a team of DSPs and LPNs, ensuring regulatory compliance and a warm, home-like environment while bridging medical needs and quality of life.

The role requires an active LPN license, 3–5 years of nursing experience (IDD/geriatrics preferred), and at least 1 year of supervisory work. Strong background checks and drug screening are required.

Qualifications

• Active, unencumbered LPN license in this state.
• 3–5 years of nursing experience, preferably in IDD, geriatrics, or assisted living.
• At least 1 year of supervisory or management experience.
• Ability to pass a comprehensive background check and drug screening.

Responsibilities

• Clinical Oversight & Care Coordination: oversee medical needs, medication administration, wound care, and chronic disease management; update ISPs.
• Staff Leadership & Training: hire, train, supervise LPNs and DSPs; provide training on Med Tech, infection control, and health protocols.
• Compliance & Quality Assurance: audit MARs and nursing notes; lead state audits/inspections and risk management initiatives.
