Qualifications

• High School Diploma or equivalent; Associate's Degree preferred.
• Active NYS Licensed Practical Nurse (LPN) license.
• Current CPR certification.
• Supervisory or leadership experience preferred.
• Experience in long-term care or skilled nursing preferred.
• Strong clinical assessment, communication, and organizational skills.
• Ability to lead, mentor, and support nursing staff.
• Compassionate, resident-centered care.

Responsibilities

• Supervise nursing staff and support the delivery of high-quality resident care.
• Administer medications, treatments, and nursing services in accordance with physician orders and facility policies.
• Monitor resident conditions and ensure appropriate interventions, documentation, and communication occur.
• Collaborate with residents, families, physicians, and interdisciplinary team members to support individualized care plans.
• Provide coaching, training and mentorship to nursing staff to promote professional growth and teamwork.
• Ensure compliance with state and federal regulations, infection control practices, and facility policies.
• Maintain accurate documentation of resident care, treatments, and clinical outcomes.
• Address clinical concerns, staffing issues, and resident care needs to ensure continuity of care.
• Perform additional duties as assigned.
