Job summary

Garden Gate Health Care Facility in Cheektowaga, NY is seeking a Licensed Practical Nurse (LPN) to join our nursing team. You will provide high-quality nursing care, administer medications, and support resident well-being in a team-focused environment.

Shift: evenings, 3 pm – 11 pm. Full-Time and Part-Time options available. Must hold a NY LPN license and CPR certification.

Qualifications

• High School Diploma or equivalent required; Associate's Degree preferred.
• Current NYS LPN license required.
• CPR certification required.
• Experience in a skilled nursing facility or geriatric population preferred.
• Strong clinical, communication, and organizational skills.
• Ability to supervise nursing assistants and work effectively in an interdisciplinary team.

Responsibilities

• Provide nursing care in accordance with resident assessments, care plans, and facility policies.
• Administer medications, treatments, enteral feedings, and IV therapies within the LPN scope of practice.
• Monitor resident conditions and promptly communicate changes to providers, supervisors, and responsible parties.
• Accurately document care, treatments, observations, and medication administration.
• Supervise and support Nursing Assistants to ensure quality resident care.
• Maintain up-to-date resident care information and communicate pertinent information during shift changes.
• Follow infection control, safety, resident rights, and confidentiality standards.
• Participate in interdisciplinary care planning and quality improvement initiatives.
• Assist with admissions, transfers, and other nursing responsibilities as assigned.

REQUIREMENTS:

• High School Diploma or equivalent required; Associate's Degree preferred
• Valid NYS Licensed Practical Nurse (LPN) License required
• Current CPR Certification required
• Experience in a skilled nursing facility or with the geriatric population preferred
• Strong clinical, communication, and organizational skills
• Knowledge of medication administration, infection control, and resident care practices
• Ability to supervise nursing assistants and work effectively as part of an interdisciplinary team
