Qualifications

• Must have NYS LPN license.
• Current CPR certification required.
• Supervisory or leadership experience preferred.
• Experience in long‑term care or skilled nursing preferred.

Responsibilities

• Supervise nursing staff and support delivery of high-quality resident care.
• Administer medications, treatments, and nursing services per physician orders and policies.
• Monitor resident conditions and ensure appropriate interventions and documentation.
• Collaborate with residents, families, physicians, and interdisciplinary team for individualized care plans.
• Provide coaching, training, and mentorship to nursing staff for growth and teamwork.
• Ensure compliance with state and federal regulations, infection control practices, and policies.
• Maintain accurate documentation of resident care and outcomes.
• Address clinical concerns, staffing issues, and resident care needs to ensure continuity of care.

Requirements

• High School Diploma or equivalent required; Associate’s Degree preferred
• Valid NYS Licensed Practical Nurse (LPN) License required
• Current CPR Certification required
• Supervisory or leadership experience preferred
• Experience in long‑term care or skilled nursing preferred
• Strong clinical assessment, communication, and organizational skills
• Ability to lead, mentor, and support nursing staff
• Strong problem‑solving abilities and attention to detail
• Commitment to providing compassionate, resident‑centered care
