Job summary

Garden Gate Health Care Facility is hiring an LPN in Cheektowaga, NY. The role involves providing nursing care, administering medications, and ensuring resident safety with a team of caregivers.

Evening shifts from 3 pm to 11 pm are available as Full-Time or Part-Time positions, with shift differential. Experience in geriatric care is preferred, CPR required.

Qualifications

• High School Diploma or equivalent; ADN/Associate's preferred.
• Valid NYS LPN license required.
• Current CPR certification.
• Experience in skilled nursing or geriatric care preferred.
• Strong clinical, communication and organizational skills.
• Knowledge of medication administration and infection control.

Responsibilities

• Provide nursing care per resident assessments and care plans.
• Administer medications, treatments and IV therapies within scope.
• Monitor resident conditions and notify providers of changes.
• Document care and medication administration accurately.
• Supervise and support Nursing Assistants during shifts.
• Communicate during shift changes and maintain care information.
• Follow infection control and resident rights standards.
• Participate in interdisciplinary care planning and improvements.
• Assist with admissions and transfers as assigned.

Rate: $27.54- $32.77/hour (depending on experience)

Responsibilities

• Provide nursing care in accordance with resident assessments, care plans, and facility policies
• Administer medications, treatments, enteral feedings, and IV therapies within the LPN scope of practice
• Monitor resident conditions and promptly communicate changes to providers, supervisors, and responsible parties
• Accurately document care, treatments, observations, and medication administration
• Supervise and support Nursing Assistants to ensure quality resident care
• Maintain up-to-date resident care information and communicate pertinent information during shift changes
• Follow infection control, safety, resident rights, and confidentiality standards
• Participate in interdisciplinary care planning and quality improvement initiatives
• Assist with admissions, transfers, and other nursing responsibilities as assigned

Requirements

• High School Diploma or equivalent required; Associate's Degree preferred
• Valid NYS Licensed Practical Nurse (LPN) License required
• Current CPR Certification required
• Experience in a skilled nursing facility or with the geriatric population preferred
• Strong clinical, communication, and organizational skills
• Knowledge of medication administration, infection control, and resident care practices
• Ability to supervise nursing assistants and work effectively as part of an interdisciplinary team
• Compassionate, resident-centered approach to care
