Job summary

Avamere Rehab of Oregon City is seeking a Licensed Practical Nurse (LPN) for a PRN Saturday day shift (6am–6pm) at 1400 Division St, Oregon City, OR 97045. You will provide basic nursing care, monitor vital signs, administer medications under RN supervision, and support residents with daily activities.

The role requires an active LPN license in Oregon, CPR/BLS certification, and preferably experience in long-term care or geriatric rehab.

Qualifications

• Must possess a nursing degree from an accredited college or university.
• Must possess a current, active LPN license in the state.
• CPR/BLS certification required during employment.
• Experience in long-term care, geriatric rehab, sub-acute or critical care preferred.

Responsibilities

• Provide basic nursing care and administer medications under RN supervision.
• Monitor vital signs and document observations accurately.
• Wound care including dressing changes and infection checks.
• Assist residents with hygiene, dressing, bathing and mobility.
• Ensure proper nutrition and hydration; assist feeding as needed.
• Record patient history and changes; communicate with the RN and team.
• Maintain patient safety and infection control; document all care.
• Supervise CNAs under RN guidance.
