Job summary

Alternate Solutions Health Network is seeking a Licensed Practical Nurse (LPN) to provide home-based nursing care to patients under physician orders. You will work with an interdisciplinary team, manage time autonomously, and support patients and families through recovery in the home setting.

The role emphasizes compassionate communication, adherence to state practice guidelines, and efficient, safe care delivery.

Qualifications

• Licensed Practical Nurse with current license in the state of employment.
• Minimum one-year experience as a LPN in an acute care setting.
• Home care experience preferred.
• Valid driver''s license and auto insurance in your name as a driver.
• Capable of all physical demands.

Responsibilities

• Plan of Care: Provide skilled nursing care as ordered by the attending physician and observe patient conditions.
• Minimize Patient Risk: Administer medications as prescribed and educate patients to prevent rehospitalizations.
• Family Educator/Advocate: Educate patient and family on disease process using teach back methods.
• Policies: Complete all clinical documentation following agency protocols and Medicare/Federal guidelines.
• Rules and Regulations: Follow agency policies and communicate schedule changes to schedulers.
• Operations: Attend in-service training and mandatory agency meetings.

HOW YOU'LL MAKE A DIFFERENCE

You'll be part of an interdisciplinary team that focuses on providing compassionate quality care and producing positive outcomes for your patient population. You'll provide home-based nursing care for patients as directed by the attending physician and by following the order sets and goals created by the Case Manager. HOW YOU'LL WORK

You'll provide skilled nursing care in a patient's home as directed by the attending physician. You will work autonomously, so you need to be efficient in managing your time. You'll be responsible for all practices and duties within the scope of practice outlined by the state.

MAJOR AREAS OF RESPONSIBILITY

Plan of Care: Provide skilled nursing care as ordered by the attending physician and observe and monitor patient conditions and notify case manager of any changes in the patient’s condition. Minimize Patient Risk: Administer medication as prescribed by the physician and help decrease re-hospitalizations by educating the patient on preventative measures and making good decisions. Family Educator/Advocate: Educate the patient and family on the disease process using teach back methods to ensure patient and family understanding. Policies: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines. Rules and Regulations: Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors. Operations: Attend in-service training and mandatory agency meetings.
