Job summary
PACS in Westminster, Colorado is seeking a dedicated Licensed Practical Nurse (LPN) to join our Skilled Nursing & Rehabilitation team. You will provide hands-on care, implement care plans, and collaborate with RNs, CNAs, and therapists to support residents in recovery and daily living.

Qualifications
- Active LPN license in good standing.
- Skilled nursing or rehab experience preferred (new grads welcome!).
- Strong clinical skills and attention to detail.
- Excellent communication and teamwork abilities.
- Heart for serving seniors and those in recovery.
Responsibilities
- Provide direct nursing care per care plans and physician orders.
- Administer medications and treatments accurately and safely.
- Monitor residents’ conditions and report changes promptly.
- Collaborate with RNs, CNAs, therapists, and physicians.
- Support rehabilitation goals and promote patient independence.
- Deliver compassionate, resident-centered care every shift.
