Job summary

Summit Ridge Skilled Nursing is seeking a Licensed Practical Nurse to lead daily nursing activities, supervise LPNs and CNAs, and ensure holistic, compliant resident care in a 24-hour setting. The role involves medication administration, care planning, and collaboration with the interdisciplinary team to meet resident needs.

The LPN must hold an unrestricted LPN license, CPR certification, and be prepared to participate in admissions, discharges, and end-of-life care as required.

Qualifications

• CPR certification required.
• Experience in long-term care preferred but not required.
• Current unrestricted LPN license in practicing state.

Responsibilities

• Directs day-to-day nursing activities for LPNs and CNAs.
• Follow nursing model to provide holistic care.
• Ensure policy compliance by nursing staff.
• Participate in admissions, discharges, and transfers.
• Transcribe physician orders and administer medications per orders.
• Collaborate with interdisciplinary team to meet resident needs.
• Promote teamwork and effective communication.
• Assist in evaluation of staff performance as directed.
• Maintain infection control and safety standards.
