RYZE at the Ridge is seeking a dedicated Licensed Practical Nurse (LPN) to join our compassionate care team at our skilled nursing facility. As a licensed professional, you will provide total nursing care to residents on the assigned unit and supervise team members.

The role emphasizes developing individualized care plans, administering medications, documenting care, and staying current with facility policies.

Qualifications

• Graduation from an accredited nursing education program.
• Active Illinois LPN license.
• No prior abuse convictions.
• Strong communication and teamwork skills.
• Ability to follow policies, maintain safety, and deliver empathetic care.
• Must work with diverse resident populations, including elderly and mentally ill individuals.
• Must be physically able to perform nursing duties with or without accommodations.

Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift as an LPN.
• Develop and implement individualized care plans with the healthcare team.
• Administer medications and treatments as prescribed and evaluate effectiveness.
• Maintain an environment that promotes residents’ well-being.
• Educate residents and families on care plans and procedures.
• Monitor team member performance and assist with development and evaluation.
• Delegate and ensure completion of nursing tasks to team members.
• Stay current with facility policies and ongoing nursing education.
• Recognize changes in resident conditions and act accordingly.
• Document care and responses per facility guidelines.
• Act as a charge nurse: answer questions and support resident care decisions.
• Follow all facility safety policies and procedures.
