Job summary

BrightStar Care Plano/North Dallas is seeking an experienced LPN/LVN for in-home, skilled nursing with wound vac focus. The role offers part-time, flexible scheduling as you care for patients in their homes across Prosper, TX and surrounding areas.

You’ll work with a dedicated clinical team, manage wound vac care, administer medications, and document care plans. The position emphasizes independence, patient advocacy, and high clinical standards within a supportive home-health environment.

Qualifications

• Current Texas LVN/LPN license in good standing.
• Hands-on experience with wound vac management.
• Home care or skilled nursing experience preferred.
• Strong clinical assessment and communication skills.
• Able to work independently while staying connected to the clinical team.
• Excellent organization and documentation skills.
• Reliable transportation, valid driver’s license, and current auto insurance.

Responsibilities

• Provide skilled nursing care in the patient’s home.
• Manage, monitor, and provide care related to wound vac therapy.
• Administer medications and treatments as ordered.
• Monitor and assess the patient’s condition and report changes appropriately.
• Maintain accurate and timely clinical documentation.
• Follow the patient’s individualized plan of care and physician orders.
• Communicate effectively with physicians, family members, and the BrightStar Care clinical team.
• Promote patient comfort, safety, dignity, and overall well-being.
• Provide compassionate, professional care while maintaining clinical standards.

Role Overview

As an LPN/LVN – Skilled Home Care with BrightStar Care of Plano/North Dallas, you’ll provide personalized, one-on-one skilled nursing care to patients in the comfort of their own homes.

This role is ideal for an experienced LVN who is confident managing wound vacs and enjoys building meaningful relationships with patients while working independently. You’ll have the support of a dedicated clinical team while maintaining the flexibility to work part-time around your availability.

Key Responsibilities
