Responsibilities

• Assist in gathering data to assess the physical, psychological, social, spiritual needs of patients.
• Collaborate with the registered nurse and team in planning for patient needs at admission.
• Review care plans and provide input into updating and revising.
• Support direct patient care utilizing clinical nursing skills and judgement.
• Provide education to patients and family and document outcomes in the medical record.
• Initiate emergency measures when needed and prepare patients for tests and procedures.
• Administer and document medications according to policy; delegate care to non-licensed staff when appropriate.
