Job summary

Grace Management, Inc. in Chicago, IL is seeking a Licensed Practical Nurse (LPN) for 2nd and 3rd shifts to supervise the wellness department on their shift and coordinate resident care. The role is on-site with full-time and part-time options depending on the shift.

The candidate will administer medications, document MAR entries, and lead RCAs and MTs. Required: nursing license in the state, and at least two years of nursing experience; English proficiency is essential.

Qualifications

• Graduate of accredited school of nursing.
• Must be a licensed nurse in the state where you work.
• Minimum of two years nursing experience.
• Ability to read, write, and speak in English.
• Experience in an Assisted Living setting preferred.
• Dementia care experience preferred.

Responsibilities

• Plans and delegates shift tasks to ensure coverage.
• Coordinates resident care and supervises RCAs/MTs.
• Administers medications per MAR and documents accurately.
• Reports changes to physician and maintains charts.
