Job summary

The Hallmark in Chicago, IL is seeking a Licensed Practical Nurse (LPN) for 2nd and 3rd shifts. The role includes supervising shift, coordinating resident care, delegating tasks to RCAs and MTs, administering medications, and ensuring MAR documentation.

Qualifications

• Graduate of accredited nursing program.
• Must have current LPN license in the state of practice.
• Minimum of two years nursing experience.
• Ability to read, write, and speak English.
• Assisted Living or dementia care experience preferred.
• In good health; ability to lift up to 40 pounds.

Responsibilities

• Plans and delegates work assignments for shift coverage and coordinates resident care.
• Supervises delivery of personal care to residents.
• Administers medications according to physician orders and MAR.
• Documents medications administered and MAR updates.
• Conducts change of shift handoffs and reports to Wellness Director.
• Maintains security of medication cart and narcotics keys.
• Transcribes physician orders and notifies pharmacy of changes.
• Responds to emergencies in a timely, calm manner.
