Discovery Village at Alliance Assisted Living and Memory Care in Fort Worth, TX, is seeking a Licensed Practical Nurse to deliver compassionate, high-quality nursing care aligned with individualized care plans. You’ll monitor resident health, administer meds, and coordinate with the interdisciplinary team to ensure safety and well-being.

The role requires a current LPN license and at least 2 years of nursing experience, with flexibility for nights and weekends.

Qualifications

• Current LPN license or RN eligibility per state regulations.
• Minimum of two years of licensed nurse experience.
• Experience in assisted living, long-term care, or skilled nursing preferred.
• Strong clinical judgment, problem-solving, and decision-making skills.
• Excellent organizational, time management, and communication skills.

Responsibilities

• Provide nursing care to residents according to established care plans, policies, and procedures.
• Conduct ongoing assessments of residents’ physical and psychosocial status and monitor for changes.
• Administer medications and treatments as prescribed and document appropriately.
• Coordinate and communicate resident care needs with physicians, families, pharmacies, and other care providers.
• Support health promotion, disease prevention, and wellness initiatives within the community.
• Ensure accurate, timely, and compliant clinical documentation.
• Assist with care planning, service plan updates, and resident evaluations as required.
• Collaborate with caregivers, wellness team members, and leadership to ensure high-quality care delivery.
• Respond to resident concerns, incidents, and emergencies using sound clinical judgment.
• Support training and guidance for care staff as appropriate.
