Signature HealthCARE is seeking a dedicated LPN/LVN to provide direct resident care, including medications and wound care, in a 112-bed rehab and long-term care setting.

The role requires at least 1 year of clinical nursing experience (long-term care preferred) and supervisory experience is a plus. Proficiency with EHR systems and MS Office is desirable. Excellent communication and independent judgment are essential.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum one year of clinical nursing experience; LTC preferred.
• Experience in a supervisory or charge nurse role preferred.
• Familiarity with electronic health records (EHR) and Microsoft Office.

Responsabilités

• Provide direct nursing care including medications, wound care, and procedures.
• Supervise nursing assistants and medication aides on shift.
• Monitor residents’ status and notify physician of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Document accurately per policy and coordinate admissions/transfers/discharges.
• Participate in quality assurance and staff education activities.
