StoneBridge DeSoto is seeking a compassionate Licensed Practical Nurse (LPN) to join our care team in De Soto, MO. This role focuses on elderly resident care within a supportive medical team, providing medications, assessments, and daily nursing tasks.

The ideal candidate holds an active LPN license, CPR certification, and preferably 2 years in long-term care, with basic EMR skills and a willingness to collaborate with staff to deliver high-quality resident care.

Qualifications

• Must hold an active LPN license and CPR certification.
• Experience in gerontological nursing preferred.
• 2 years in long-term care preferred.
• Basic computer skills and EMR experience recommended.

Responsibilities

• Coordinates the nursing care of residents on the unit.
• Uses independent judgment in performance of duties.
• Provides safe and accurate Medication related interventions to residents.
• Administers and documents medications and treatments per schedule.
• Knows the rationale for effects of medications and treatments.
• Performs nursing assessments and notifies physician of status changes.
