Job summary

StoneBridge DeSoto is seeking a compassionate Licensed Practical Nurse (LPN) to join our senior-care team in De Soto, Missouri. The role provides basic medical support and medication administration within a resident-centered setting.

You will document care in EMR, follow physician orders, and support residents across rehabilitation and memory care. Requirements: active LPN license, CPR certification, and 2 years in long-term care; strong empathy and teamwork a must.

Qualifications

• Must hold a current LPN license in the state.
• CPR certification required and additional nursing specialty certification desired.
• Prefer 2 years of experience in a long-term care setting.
• Basic computer skills and experience with Electronic Medical Records (EMR) preferred.

Responsibilities

• Coordinates nursing care for residents and ensures appropriate nurse aide support.
• Administers and documents medications and treatments per schedule.
• Provides safe, compassionate care and responds to resident requests promptly.
• Maintains accurate nursing assessments and communicates changes to physicians and team.
