Job summary

Qualifications
- Active, unencumbered LPN license in the state of practice.
- Supervisory experience preferred.
- Strong communication skills essential.
- Understand, respect and maintain resident confidentiality.
Responsibilities
- Deliver safe, effective, and compassionate nursing care to residents in accordance with individual care plans.
- Administer medications and treatments accurately and safely.
- Monitor health conditions; report changes to RN/provider and family as needed.
- Document care, observations, and assessments in the EHR.
- Provide guidance and supervision to care providers and RA’s.
- Communicate effectively with physicians, families, and the care team.
- Participate in care plan meetings and quality improvement activities.
