Job summary

Lutheran Life Communities in Crown Point, IN is seeking an LPN for part-time, day/evening shifts at Wittenberg Village to care for residents who have made this place their home. You will administer medications, check vitals, and coordinate with a familiar care team to ensure individualized routines are followed.

The role requires Indiana LPN licensure, CPR/First Aid certification, and the ability to pass background checks and drug screens.

Qualifications

• Current Indiana LPN license.
• CPR and First Aid certification.
• Able to pass a background check and drug screen.
• At least 1 year of experience in senior living, assisted living, or healthcare setting (preferred).
• Experience with electronic health records (EHR) (preferred).
• Customer service background (preferred).

Responsibilities

• Administer medications and treatments, and keep accurate documentation.
• Check and record vital signs, weight, and other resident measures.
• Support residents with daily living needs alongside the care team.
• Communicate with physicians, families, pharmacy, and vendors on resident care.
• Help build and update care plans, and follow up on resident needs.
• Watch for changes in resident condition and respond quickly.
• Keep the care environment safe, clean, and organized.
