Job summary

Cardinal Ritter Senior Services, a ministry of Catholic Charities, seeks an LPN to provide skilled nursing in our SNF facilities and supervise CNAs/CMTs. You will monitor resident care, administer medications, and ensure regulatory compliance with state/federal guidelines.

Qualifications

• Must be a licensed LPN in good standing.
• At least 4 years experience in long-term care.
• Familiar with state/federal guidelines for Assisted Living Facilities.
• Electronic Health Record literate.

Responsibilities

• Uses nursing process to assess, plan, intervene, and evaluate resident needs.
• Supervises and mentors CMTs and CNAs to ensure high-quality care.
• Promotes individualized care and monitors residents physically, emotionally, and spiritually.
• Administers medications and treatments per physician orders and policy.
• Documents resident condition and reports to Nursing Supervisor and physician.
• Conducts rounds to monitor staff and ensure timely call light response.
• Completes MDS timely and attends care conferences as scheduled.
• Maintains accurate nursing records and follows procedures for supplies.
