All American Assisted Living at Wareham seeks an LPN to provide resident-centered nursing support for older adults in a temporary role with potential to become permanent. You will administer medications, monitor health, document care, and collaborate with families and providers under nursing leadership in Massachusetts.

The position emphasizes person-centered care, safety, and adherence to MA LPN scope, with opportunities for training and professional growth in a supportive team environment.

Qualifications

• Massachusetts LPN license active and in good standing.
• Knowledge of practical nursing standards and medication administration.
• Strong observation and resident-monitoring skills.
• Strong documentation and organizational abilities.
• Ability to communicate clearly in English, both written and spoken.

Responsabilités

• Administer medications and treatments per provider orders and nursing standards.
• Monitor residents for changes in health, cognition, mood, and function.
• Provide direct nursing care within MA LPN scope of practice.
• Document health information accurately and promptly.
• Participate in resident assessments and service-plan reviews.
• Develop, implement, evaluate, and update resident care plans.
• Report significant changes promptly to nursing leadership.
• Coordinate medication refills with pharmacies and providers.
• Support admissions, transitions, and changes in level of care.
• Educate residents and families within scope of practice.
• Promote wellness, hydration, nutrition, infection prevention, and safety.
• Respond to emergencies and urgent needs promptly.
• Attend resident-care meetings and communicate updates to Wellness team.
• Provide supervision to resident-care staff as assigned.
• Delegate nursing activities within legal and policy constraints.
• Assist with staff training and in-service education.
• Maintain resident privacy, dignity, and rights.
