Job summary

Kaplan Senior Living in Massachusetts is seeking a compassionate LPN to join our Wellness team. You will provide resident-centered nursing care, administer medications, monitor health, and coordinate services under the guidance of the nursing leadership.

This temporary role offers meaningful work with older adults and may lead to permanent placement. Applicants must hold an active MA LPN license, be able to work within the LPN scope, document accurately, and communicate clearly with residents,

Qualifications

• Must hold active Massachusetts LPN license.
• Knowledge of nursing standards, medication administration, and resident safety.
• Strong observation and resident-monitoring skills.
• Ability to document and report changes clearly and promptly.
• Excellent communication with residents, families, providers, pharmacies, and coworkers.
• English proficiency in speaking and writing to understand orders and document care.
• Ability to supervise resident-care staff and provide direction.
• Capable of working independently with escalation when needed.
• Experience with older adults preferred (assisted living, memory care, long-term care, home care).
• Dependable attendance and punctuality.
• Must be able to stand, walk, bend, reach, and remain active during shifts.
• Ability to lift/transfer as per policy.

Responsibilities

• Administer medications and treatments according to orders and procedures.
• Monitor residents for changes in health, cognition, behavior, mood, and functioning.
• Provide direct nursing care within the MA LPN scope.
• Collect and document resident health information.
• Participate in assessments and service-plan reviews with nursing leadership.
• Develop and update resident care and service plans.
• Report significant changes promptly.
• Coordinate medication refills with pharmacies and providers.
• Document medications, treatments, and nursing services accurately.
• Support admissions, transitions, and level-of-care changes.
• Provide resident and family teaching within scope and competency.
• Promote wellness, hydration, nutrition, infection prevention, and safety.
• Respond to emergencies and urgent needs.
