Job summary

Mercy LIFE, a member of Trinity Health PACE, seeks a Licensed Practical Nurse for its West Springfield, MA location. Working full-time, Monday through Friday with an on-call rotation, you will support participants in care planning, medication management and home visits as part of the PACE model.

Under the Clinic Supervisor, you will observe and document participant conditions, implement physician orders, educate participants and families, and coordinate with the care team to deliver

Qualifications

• Graduate of a state accredited hospital or collegiate nursing program.
• Licensed Practical Nurse licensed in Massachusetts.
• Two years’ experience including one year with frail or elderly.
• Excellent assessment skills; strong interpersonal and team coordination.

Responsibilities

• Assist in treatment, examination and testing of participants under supervision.
• Observe, record and report participant condition and response to treatments.
• Coordinate appointment bookings and prioritize emergencies in care planning.
• Instruct and educate participants and families regarding medications and care plans.
• Maintain participant records, charts and related documents per Mercy LIFE policy.
