Mercy LIFE, a Mercy Health affiliate in West Springfield, MA, is seeking a Licensed Practical Nurse for a full-time position.

The role supports the PACE model, delivering comprehensive community-based care to older adults. You will work Monday through Friday with an on-call rotation, assisting in treatments, documenting care, and educating patients and families. Mercy LIFE emphasizes dignity, independence, and quality of life for participants.

Qualifications

Graduate of a state accredited nursing program with MA LPN license.
Two years’ experience, including one year with frail or elderly.
Excellent assessment skills; leadership and interpersonal abilities.
Experience in interdisciplinary care planning beneficial.
Ability to perform essential job functions safely.

Responsibilities

Under supervision of the Clinic Supervisor, assist in treatment, examination and testing.
Observe, record, and report participant condition and reactions to treatment.
Schedule appointments and manage home/urgent visits.
Educate participants and families on medications and care instructions.
Maintain and review participant records and care plans.
Provide on-call coverage as part of a rotating schedule.
Deliver restorative nursing care and health counseling.
Perform home visits as needed.
Provide Clinic coverage as needed.
