Job summary

Infinite Medical P.C. is seeking a responsible Licensed Practical Nurse to review patient charts and ensure that records accurately reflect conditions and treatment needs. You will work to maintain high-quality documentation and support care planning across care teams.

The role emphasizes comprehensive patient assessments, case management, and coordination to optimize outcomes for residents in long-term care settings. Strong communication and geriatric health experience are valued.

Qualifications

• LPN license required.
• Minimum of 2 years of experience in acute care or managed care setting preferred.
• Proficient in conducting patient assessments and monitoring vital signs.
• Excellent communication and interpersonal skills to collaborate with healthcare professionals.
• Geriatric behavioral health experience preferred.

Responsibilities

• Conduct comprehensive assessments of patients' medical, social, and emotional needs
• Provide case management services to ensure continuity of care and appropriate utilization of resources
• Monitor and evaluate patient progress, adjusting care plans as needed
• Collaborate with interdisciplinary teams to optimize patient outcomes
• Overseeing patient EMR to ensure that patients receive proper care
• Complete reports to reduce patient gaps and hospitalizations.
