You will review charts for accuracy and ensure documentation aligns with patient conditions and care plans. The role emphasizes collaboration with interdisciplinary teams, conducting thorough assessments, monitoring vital signs, and helping optimize care for residents in skilled nursing and long-term care settings.

Qualifications

• LPN license required.
• Minimum of 2 years of experience in acute or managed care preferred.
• Proficient in conducting patient assessments and monitoring vital signs.
• Excellent communication and interpersonal skills to collaborate with healthcare professionals.
• Experience with geriatric behavioral health preferred.

Responsibilities

• Conduct comprehensive assessments of patients' medical, social, and emotional needs.
• Provide case management services to ensure continuity of care and appropriate utilization of resources.
• Monitor and evaluate patient progress, adjusting care plans as needed.
• Collaborate with interdisciplinary teams to optimize patient outcomes.
• Overseeing patient EMR to ensure that patients receive proper care.
• Complete reports to reduce patient gaps and hospitalizations.
