Job summary

Infinite Medical P.C. in Decatur, GA is seeking a Licensed Practical Nurse to assist with reviewing patient charts and ensuring that information reflects the conditions and treatment needs of each patient.

You will work with interdisciplinary teams to monitor patient progress, support continuity of care, and help reduce hospitalizations while focusing on compassionate, patient-centered care for seniors.

Qualifications

• LPN license required.
• Minimum of 2 years of experience in acute care or managed care setting preferred.
• Proficient in conducting patient assessments and monitoring vital signs.
• Excellent communication and interpersonal skills to effectively collaborate with healthcare professionals.
• Experience with geriatric behavioral health preferred.
• Computer work including keyboard and mouse use for extended periods. Lifting/moving patients. Prolonged sitting.

Responsibilities

• Conduct comprehensive assessments of patients' medical, social, and emotional needs.
• Provide case management services to ensure continuity of care and appropriate utilization of resources.
• Monitor and evaluate patient progress, adjusting care plans as needed.
• Collaborate with interdisciplinary teams to optimize patient outcomes.
• Overseeing patient EMR to ensure that patients receive proper care.
• Complete reports to reduce patient gaps and hospitalizations.
